ALL COPAYS ARE DUE AT THE TIME OF APPT.

Less than 24 hour cancellation and No show: $50 first time and $100 for subsequent Less than 24 hour cancellations and no shows within a calendar year.

Screenings pertinent to treatment will be completed & billed to Insurance, however, client is responsible for payment if insurance will not cover:
